High Tech Patient DataThe following simple steps ensures that the patient’s data is of high quality by requiring the patient to review their data periodically. ┬áThe patient inserts their Harvest MD Card and enters their PIN. ┬áThe provider staff prints the data.

The patient reviews demographic, contact, insurance and personal health data stored on their Harvest MD Card to ensure that the data is accurate and complete. The patient can requests additions or changes at this point.

The patient returns the changes to the provider staff and enters their PIN to authorize the the staff to update the patient’s card.

This currently occurs by the patient having to constantly fill out the “dreaded” form. Using the Harvest MD Card eliminates the manual form.

The Harvest MD Card stores the following types of personal data:

  • Current Address & Contact
  • Permanent Address & Contact
  • Employment
  • Guarantor Address, Contact, Relationship & Employment>
  • Power of Attorney Contact
  • Emergency Contact
  • Next of Kin Contact
  • Guardian Contact
  • Personal Data such as Date of Birth, Student Status, Marital Status, Spouse Name
  • Blood Type, Transfusions and any Reactions
  • Personal Health
  • Family History
  • Advanced Directive
  • Disabilities
  • Allergies
  • Inoculations
  • Over the Counter Medicine

Of course, should the patient data ever be lost, recovery services can be contracted to retrieve the data at a very reasonable cost.
